Financial Aid Form

Dear Parents,

One of the jobs of the Board of Directors is to keep the school financially sound. To help them make wise financial decisions affecting the upcoming school year, the Board of Directors requires the following registration agreement to be read and signed by a parent or guardian. In this manner, the Board can verify that all parents are aware of the payment schedules at Madison Academy. We are striving to work together to continue maintaining "Excellence in Education."

MADISON ACADEMY STUDENT REGISTRATION AGREEMENT

In order to reserve a place for your child _______________________, for the 2013-2014 school year, the following non-refundable registration fee is due by Tuesday, April 2, 2013 3K-8th $150.00. If paid after April 10, 2013 $200.00

This amount will be applied as the 2013-2014 Book and Supply.

If the fee is not paid by April 10, 2013 your child (ren)'s slot will be available for open enrollment.

Tuition will be set at the April meeting of the Madison Academy Board of Directors and will be based on the number of returning students; therefore, it is important to complete and return the registration in a timely manner.

Tuition may be paid in monthly installments beginning on June 1, 2013 and continuing through May 1, 2014.

Tuition is due on the 1st of each month and late if paid after the 10th of the month.

A $25 late fee will be due if payment is made after the 10th of each month.

Tuition does not include the Book and Supply fee of $150 per student.

Tuition does not include any Building Fund balance.

Please mail all summer payments to the school.

Outstanding balances in arrears for more than two months may result in your child(ren)'s dismissal from the school or collection measures will be taken at the discretion of the Board.

In accordance with the philosophy and objectives of Madison Academy, a student may be asked to withdraw from the school due to academic and/or behavioral problems.

Please return this registration agreement, signed and notarized, along with your registration fee by April 2, 2013, to reserve a place for your child(ren). For your convenience, Mrs. Blair, in the office, is a notary, but the document must be signed in her presence.
